It just confirms your lh surge. The only way to confirm O is with temping. U can get a pos. opk and not O. U can also get a neg. opk but temp rise which means u did O. It is possible your body prepared to O but didn't. I looked up this question earlier today, actually. I am having a similar thing; fatigue, o-pains, acne, +opk's, temp low, and I mean what my temp is usually w/o the clomid and progesterone. What I found out was that you can have +opk's and still not ovulate. Anyway, you can have the LH surge and not ovulate if your egg isn't fully ready to let go.I am still in shock - I got a bfp today. I was desperately hoping I would get af soon (I have only had one pp period so far) so we could officially start ttc with all the bells and whistles (opk, bbt, supplements, etc). I was actually excited for the cramps I have been feeling - I figured she was right around the corner! I don't know how long I will get to hang around here - but for the moment ....
